These forms were developed by Club President Nancy during her first term of office. They were presented in booklet form to the District Governors during their visits. Each club member received one as well. This is meant as a guide to help you develop your own booklet if you so desire. Maysville members found this extremely helpful as all club information was at their fingertips. You are welcome to download the samples below and use them at your descretion.
